AgeWell Transportation

It is the mission of AgeWell Physical Therapy and Wellness P.C. to provide quality outpatient physical therapy services to the geriatric population. AgeWell recognizes that transportation can make accessing quality healthcare prohibitive to some individuals seeking treatment. It is the policy of AgeWell to provide transportation free of charge, to those individuals that qualify for this service.

Conditions:

In order to qualify for free transportation, riders must be over 65 years old and demonstrate that they do not drive and that there are no other drivers living in their domicile. If the patient does not meet this criterion they may still be eligible to arrange transportation.

Limitations:

AgeWell will accommodate all patients' transportation needs to the best of its ability; however, riders must be aware of the following limitations:

  • Riders must understand that appointment times will be limited by the vehicles availability.
  • Riders must understand that their appointments may have to be changed due to changes in the driving schedule.
  • Riders must understand that pick-up and drop off times may vary due to traffic conditions and other unforeseen delays.
  • Riders must understand that the vehicle may stop to pick-up or drop off other riders while they are in the vehicle.
  • Riders must understand that transportation may become unavailable for an unknown period of time and without warning (i.e. driver unavailable, mechanical problems, etc…)

Please Note:

All persons in vehicle must comply with wearing seatbelts, this includes the back seats also, the drivers are instructed not to proceed until all comply;

Gratuity: In the past patients have asked about gratuities for the driver. Gratuities are not required or even expected. If you choose to tip the driver, however, a gratuity should only be considered if you feel you were provided with excellent service.

From the west:

Take the Long Island Expressway east to exit 33 = Lakeville Road / Community Drive. Make a right onto Lakeville Road and head south to Marcus Avenue. Make a left on Marcus Avenue. Follow Marcus Avenue and make a right at 1999 Marcus Avenue.

[Alternate Route] Take the Grand Central Parkway east which turns into the Northern State Parkway to exit 25 = Lakeville Road / New Hyde Park Road / Great Neck. At end of ramp, make a left at the light onto Marcus Avenue. Follow Marcus Avenue and make a right at 1999 Marcus Avenue.

From the east:

Take the Long Island Expressway west to exit 34 = New Hyde Park Road. Make a left at the light onto New Hyde Park Road and head south. Make a right onto Marcus Avenue. Make a left at 1999 Marcus Avenue.

[Alternate Route] Take the Northern State Parkway west to exit 26 South. Drive South on New Hyde Park. Make a right onto Marcus Avenue. Make a left at 1999 Marcus Avenue.

Once on the property at 1999 Marcus Avenue you may park and enter the building at the main entrance.

Or you may use our private entrance one level down by driving down the ramp located on the east side of the building. At the bottom of the ramp, make a sharp u-turn to the right. The building is now on your left hand side. Drive around to the opposite side (the west side) of the building. Our door is clearly marked with the AgeWell sign. We are the second door on the west side of the building.

This entrance is recommended for persons who have difficulty walking far distances

From the North:

From the Woodside Area: Go south on 69th Street. Turn left onto Grand Avenue. Arrive at 69-61 Grand Avenue on left.

From the Elmhurst area: From Queens blvd, drive southwest on Grand Avenue. Arrive at 69-61 Grand Avenue on right.

From the South:

Drive north on Fresh Pond Road towards 71st Avenue. Turn right onto Flushing Avenue. Flushing Avenue becomes Grand Avenue. Arrive at 69-61 Grand Avenue on left.

From Manhattan:

From the West: Drive through the Queens Midtown Tunnel which becomes the Long Island Expressway (I-495 East.) Exit at Maurice Avenue. Stay straight to Borden Avenue. Turn left onto Grand Avenue. Arrive at 69-61 Grand Avenue on left.

Free TransportationFrom the East: Drive west on the Long Island Expressway (I-495E.) Take the ramp toward 69th Street/Grand Avenue. Stay straight to go onto the Queens Midtown Expressway. Turn right onto 69th place. Turn right onto Grand Avenue. Arrive at 69-61 Grand Avenue on left.
